The History of the Suture

Who came up with the idea of stitching together human skin like ripped jeans? Nick Snelling delves into medical history to find out.

Getting stitches is a rite of passage as a kid. Fall off your BMX, run into something sharp or gouge yourself leaping out of a tree, and you were off to the doc quick-smart, wincing and wailing as they sewed you back up. But did you ever stop to consider who first thought to ‘repair’ a tear in people’s skin by stitching it back together like a seamstress or tailor?

Mentions of physicians suturing human skin stretch all the way back to 3000 BC and the Ancient Egyptians. But given their penchant for opening up dead people, removing their organs and sewing them back shut (archeologists have found sutures on mummies dating back to 1100 BC), it’s still unclear whether they first got the idea for stitching up wounds on the living, or the other way around.

Nonetheless, the practice was scribbled down in detail by a famed text of the Classical Sankskrit era called the Sushrut Samhita. This tome was written in about 500 BC by the legendary Indian sage Sushruta, who even now is regarded as the granddaddy of surgery. Around about the same time, another toga-toting forefather of medicine, Hippocrates (he was the Greek guy whom the doctor’s oath is named after), laid out his own version of how to stitch clumsy people up. Copying their Ancient Greek buddies’ work as usual, the Romans followed suit a century or so later, with both the mysterious encyclopeadist Aulus Cornelius Celsus mentioning it in his dusty old tome De Medicina around 47 AD, and a hundred years on, the polymath Galen talking up sutures he successfully performed on internal gut injuries.

Up until that point and much later, however, sutures were usually carried out using fibrous plant stuffs like hemp, flax and cotton, and had to be cut and plucked out before they caused too much infection. But around about the 10th century, the fabled Andalusian surgeon Abulcasis noticed one of his pet monkeys chowing down on some lute strings. The strings were made of catgut, and when the monkey didn’t die, it occurred to the beardy Abulcasis that catgut’s dissolvable nature might make it usable for suturing wounds.

Over the next thousand years or more, while needle designs improved and new suturing patterns and knots were learned and exchanged, the materials didn’t change much. Animal products such as hair, silkthread and dried-out arterial, nerve, muscle and intestinal fibre became increasingly adopted for stitching wounds, while the needle material itself didn’t vary from either silver, copper or bronze wire.
As techniques improved, it wasn’t until the 1860s that Scottish surgeon Sir Joseph Lister introduced sterilisation of all suture threads and needles using carbolic acid. Joe was influenced by is mate Louis’ pasteurisation process, and he led a slow but sure acknowledgement among the medical fraternity of the real existence of ‘tiny microbes’. The consequences were revolutionary – not only did fewer surgery patients die from infection, but sutured wounds tended to scar less. Even more so when sterile catgut was first pulled off in 1906 by treating it with iodine.

Interestingly, controversial French physician Alexis Carrel was the first surgeon to ever develop the technique of ‘triangulation’ to reconnect severed blood vessels. Observing the work of an embroider, Carrel had been inspired when an attack on the French president Sadi Carnot by a knife-wielding assassin in 1894 saw the poor politician bleed out before his resident quacks could save him. While Carrel pioneered a new wave in vascular medicine that eventually saw him awarded a Nobel Prize, unfortunately, he also had some less savoury ideas – including a whole treatise on the value of ‘eugenics’, which popped up later and heavily informed Nazi ideology.

Fast forward, and by the 1930s, mass manufacture of several different types of synthetic sutures saw the development of absorbable stitches made of polyvinyl alcohol. By the ’50s, radiation caught on as a better means of sterilisation, ten years later, a dissolvable suture made of polyglycolic acid was invented and became universal throughout most surgeries by the 1970s. Even now, most sutures are made of polymer fibres.

A Historical Glimpse of Suturing

Suturing is commonly used today as a means by which wounds are closed in order to prevent infection and hasten healing. Stitches are used by surgeons, doctors, nurses, podiatrists, dentists, first aid workers, veterinarians and survivalists. However, where did the concept for suturing wounds begin?

Archeologists have discovered primitive needles of bone and later metal that were thought to be used for surgical suturing thousands of years ago. It is suggested that the original versions of suture threads were coarse and made from plant fibers before progressing to animal parts much later in history. The first obvious proof of using sutures surgically comes from the Egyptians who used the technique extensively for burial preparation of their mummies. It is assumed that they also used their suturing skills on living people as well. The Greek physician Hippocrates recorded his use of sutures and plant-based materials, but the crude methods and materials most assuredly led to infections and severe scarring during that time of use.

Although the exact date of the transition from plant fibers to animal suturing materials is unknown, the first recorded use of catgut as a suturing material is attributed to Galen of Pergamon in the second century AD. Catgut was and still is made from cattle and sheep intestines, although synthetic catgut is more commonly used today, but like its predecessor, does not require removal because it dissolves in the body.

One of the main problems with suturing surgical incisions and wounds early on was that sterile instruments were not used which often led to skin irritations and infections. In 1860, Joseph Lister addressed the problem by creating a method of for sterilizing suture material by using carbolic acid. Chromic acid was later used to achieve sterilization and then iodine was introduced to sterilize catgut in 1902. These substances greatly decreased the incidents of infection in sutured wounds.

Sterilized catgut has been successfully used for decades and is still in use today. Eventually, however, doctors began to seek other suture options because catgut dissolved unpredictably in different patients which sometimes led to wounds reopening. Scarring was also a major problem with catgut sutures. Synthetic suture materials were produced in the 1920’s and the 1930s saw the creation of two types of surgical sutures, absorbable and non-absorbable which offered doctors a broader choice in suturing materials.

Today, most suture kits contain suture thread made from polymer fiber materials which work well for a variety of surgical suturing procedures. Polymer fiber suture thread comes in a wide selection of styles, designs and types which can be utilized for various wounds and under numerous conditions. Due to its flexible nature, polymer fiber is the preferred choice of physicians, nurses, dentists, veterinarians and first responders and is the best selection for survivalists, adventurers, sports teams and high-risk job workers. Suturing has come a long way since its conception and suture kits offered today contain top sterile equipment and materials to perform wound stitching properly which significantly reduces both infection and scarring.

Suture Pins

Pins used to approximate the sides of a soft tissue incision, from an 1864 Tiemann surgery set.  The pins are round with a round, not a flat head.  These needles were found in the paper holder and would have been supplied this way.

In the diagram, the pins are shown with suture material wrapped  in a criss-cross on either side of the incision and tied to close the incision, rather than being passed through the tissue.

After the war, 1861 - 1865, the medical doctors from North and South came together in an attempt to exchange what they had learned. This information was to be used in the emergency treatment of civilians.

One glaring difference was the substantially higher number of ancestors who survived major surgery in the South than in the North, where the post treatment infection rate caused high numbers of
fatalities.

The doctors of the North (often called "sawbones" for all the amputations done) used imported silk thread for stitching wounds. It was strong, light and "slick" , making use much easier during
stitching.

Because of the naval blockades, there was no silk thread in the South, for even the most prominent of patients. Cotton thread was not a success as it broke down before the wound had healed
sufficiently. Horse hair was tried. But because it was too stiff to make sutures in it's natural state, it was boiled in water to soften and become more pliable.

The result was that the Northern silk thread had been handled by countless unwashed hands from several continents and introduced all the pathogens it carried right into the wound.

The horse hair was sterile when it came from boiling, and handled by significantly fewer numbers of people. Far fewer pathogens were introduced right into those wounds.

With antibiotics and sterile procedural practices still years away, many folks owe their ancestral branches to the boiled horse hair sutures, and to the women and men who devised that method of
softening the fibers for using in textiles.

Suture Needles and Suturing During the Civil War Era

Sometimes the most obvious things are sitting right under your nose.  Based on how suturing is done today, it suddenly dawned on me I had never seen a suture forceps in any of the surgical sets of my pre-1870 collection.  None!  There were lots of suture needles and suture materials (wire and thread), but no suture needle forceps!  Why?  ...Because they used their fingers to suture, just like women and tailors of the time used their fingers to sew!  That is why the curved and straight needles found in surgical sets are so large. The suture needles of today are very fine and curved along the size of a dime and require handling by sturdy needle forceps to place sutures, where as those prior to the late 1880's were curved along the size of a silver dollar or twenty dollar gold piece and sutures were placed by hand in most cases.  Now, all that being said, there were 'needle holders', just not the type we associate with suturing later in the 1900's.  Forceps with grooves in the beaks are seen that would have stabilized a suture needle.  But dedicated locking suture needle holders per se, were not commonly found in American sets.

In one Snowden and Brother Civil War surgical case, I found a Physick's forceps which is expressly used to hold a needle firmly in a grove in the jaws of the tip.  I have also seen this Physick's forceps referred to in multiple texts for deep suturing in a wound.

Horsehair as a Substitute for Wire (in sutures)

As a material for attaching the margins of the skin, and mucous membrane after circumcision, or other operations for phimosis, I have found horsehair most useful, having employed it both in children and adults. . . In the removal, the advantage of horsehair sutures over the wire is considerable, since, unlike wire, which after remaining a few days in a wound, stiffens into a metallic ring, horsehair, when cut just aside the knot, either retaining its original elasticity, springs open, or if it has been long soaked in the wound secretions, it becomes soft and pliable. I would recommend this suture for wounds of the eyelid and other parts of the face, and to the loose integuments of the scrotum and penis; since to all these parts I have either applied the suture myself with good effect, or I have seen it used by others at my suggestion.

For the purpose of suture, long white tail hairs are the best. Before being used they should be soaked for a minute or two in water, or they may be drawn once or twice through the moistened finger-ends. The suture may be fastened off in a double knot, but if the hair is stiff, a third knot if often required. It may be removed in the ordinary manner, seizing the knot with the forceps, and dividing the suture just aside of it. It is scarcely necessary to remark, that horsehair, as a suture, is not suitable for wounds where there is much tension between the edges.
